Monthly Study Submission

Please complete both Part 1 and Part 2 of the following forms. Your responses help us understand your current experience.

In Part 1, please use these definitions to indicate how often each statement applies to you:

• Rarely: Happens infrequently, with long periods between occurrences.
• Periodically: Happens from time to time, with some regularity.
• Frequently: Happens often, occurring regularly throughout a period.
• Consistently: Happens regularly or almost every time, with few exceptions.

Part 2 requires detailed feedback. We encourage you to keep a journal throughout the month to record symptoms, emotions, experiences, and other notable changes. You may use these notes to help you complete the monthly questionnaire accurately.
